You can use this form to join the CENTER for SPIRITUAL CARE and PASTORAL FORMATION and affiliate with a practitioner group, which we call a "Practitioner Community". The Practitioner Community is a small, often local group of practitioners in various stages of formation, typically groups of twelve or less. The Practitioner Community provides support to its members; activities focus on providing consults, reviewing case studies, offering didactics and networking. The Practitioner Community is also the vehicle to certification with CSCPF and also provides Readiness Review in preparation for the candidate's Certification Review Board.

Some Practitioner Communities meet locally, others virtually. We invite you to contact a "Practitioner Community Contact" listed in the Directory of Communities to arrange a visit with one of our Practitioner Communities. You can check and contact a group near you clicking on the Directory of Communities.

This information on the form below will be used in both your Practitioner Community and the CENTER for SPIRITUAL CARE and PASTORAL FORMATION ("CSCPF"). It is important to note that submission of this application constitutes your acceptance and adoption of the CSCPF Code of Ethics.
