This form supports affiliating all three types of communities with the CENTER for SPIRITUAL CARE and PASTORAL FORMATION ("CSCPF"): Practitioner Communities, Learning Centers, and Serving Sites. Submission of this application form constitutes acceptance and adoption of the CSCPF Code of Ethics by your community.

Please note that the information you provide on this form will be available publicly so that those interested will be able to contact you. As with all of our participant data, however, you have complete control over what you make public. 

PLEASE NOTE: All participants require a unique email address, so your Practitioner Community contact email address cannot be the same as that for you as an Individual Member.

Note for Practitioner Community Liaisons: All Individual Members must belong to a Practitioner Community, so please make sure your members identify your Practitioner Community accurately in their Profiles.

PLEASE NOTE: Community Member rates have NOT increased for 2017. Any new membership paid in June, July, and August 2017 will pay at the new rate and you will not have to renew again until September 2018.

TYPES OF COMMUNITIES
Learning Center - These are communities which offer pastoral formation at various levels of certification. Learning Centers must be accredited, and might be incorporated. The annual fees for Learning Centers include accreditation (or renewal of accreditation, if you are already accredited).

Practitioner Community - These are small, local groups of practitioners in various stages of formation, typically groups of twelve or less. These can be incorporated, but commonly are simple associations. The Practitioner Community provides support to its members; activities focus on providing consults, reviewing case studies, offering didactics and networking. The Practitioner Community is also the vehicle to certification with CSCPF, provides mentoring and a Readiness Review in preparation for the Certification Review Panel.

Serving Sites - These are placement sites for individual participants who are CPE interns, in initial formation, or the sites where certified participants practice. Any type of ministry environment is acceptable, once reviewed by local Learning Centers and Practitioner Communities.

Payment Terms - Fees are due on the submission of the application. If you wish to be invoiced, a standard invoice will be sent to the e-mail address of the Community Liaison provided in the profile. Invoices are due on receipt, and are payable by check or via PayPal to CSCPF-Finance@cscpf.org.
